The point is that the features you are talking about in your example are not really features of safety nets in general, and it's very specific to this one way of doing things, mainly because we didn't design the ACA as a safety net but as a way to get more people insured. I don't know, it seems like Star Trek has been all downhill since DS9. Voyager and Enterprise at least tried to keep to canon, but it's like everything they made since is about bringing the widest audience with a complete disregard for the existing universe.
