Erm, why would that be an issue, exactly? If it got played while another was in the stack, everything would happen again, yes, but it'd have no target -- nothing on the field, nothing in either player's hand.

Unless it was played and countered, in which case it would exist in the players graveyard which would cause the second copy to be exiled. It is an instant, so it could be played as a response to another player playing the same card, which would exile both copies and neither would resolve. As unlikely as that is, it could happen.

It was more of a definition than a fix, in any case (you called it a legendary instant, I defined that based on my best guess as to how WoTC would do so). The actual fix was where you wrote both and I substituted each. Remember, the game can be played with more that two people.

When ~ comes into play, put five 1/1 Voodoo Doll creature tokens into play under your control.
At the beginning of your turn, tap a non-creature permanent for each Voodoo Doll under your control.
WW, T: Sacrifice a Voodoo Doll, target creature gains +0/+4. Play this ability as a sorcery.
BB, T: Sacrifice a Voodoo Doll, target creature gains +0/-4. Play this ability as a sorcery.
