JS-B makes a valuable observation. I have personally observed people with severe injuries and little or no pain and vice versa. Several possibilities exist to explain it but the human nervous system, being as prodigiously complex as it is, so far defies understanding.
Inhibitory signals in the cord or centrally may block off pain signals - possibly under the influence of adrenaline and other stress related humoral factors. Simple inattention may explain some - too busy firing at an advancing enemy to notice your leg's blown off etc.
All doctors can relate incidents of genuine suffering in the absence of detectable pathology.
Fascinating and frustrating - masses of research still to do.

Has anyone read Ray Kurzweil's "The Singularity is Near"? It basically explains how all of the o.p.'s listed technologies along with everyone in the thread's questions, and is pretty much the best transhumanism reference out there.
