Completed now that I'm home. I'm curious though, why you called it "workplace preferences" when all of the questions seemed to be about how my workplace is, not how I would like it to be.

I did it, also, saying something is too loud implies that you'd like it to be quieter. That sounds like a preference to me, given that the survey also asks where and in what field you work.

The survey required me to answer about temperatures for all seasons but I do not work during the summer.

That's true. I was expecting questions more like "Would you prefer your workplace temperature at 40F, 60F, 80F?" as opposed to "is your workplace too cold or too hot?"

Yeah, in hindsight I would have added an option like I did for the time periods re: noise, so you could have chosen "I don't work during this season." - my bad!

I did it, but I don't think you were asking about fast food, and I don't really consider my work at the university to be...well, work, so much as an education, even if they pay me for it, so...

Completed. I've included it in the comments at the end of the survey, but I've gone with including everything I encounter in a working day. I move between several rooms, so some of them are noisier than I'd like, while others are fine; I decided to go with saying that my workplace was noisy as that is a regular occurrence.
Please note that despite the lovely avatar Sungura gave me, I am not a medical doctor.
